It is a colour vision test designed by Shinobu Ishihara in the early twentieth century, and it is still the standard first screen for red-green colour blindness.

How the plates work

Each plate is a field of dots in mixed colours, sizes and shades. A number is picked out in one set of colours against a background of another. The trick is that the figure and the ground are chosen to be similar in brightness but different in hue, along the exact colours that people with a red-green deficiency confuse.
